This is an NHS service that aims to prevent heart disease, stroke, kidney disease and specific forms of dementia. The service is available for all patients aged between 40 and 74 who do not have specific risk factors for these conditions or an existing diagnosis.

The health check service assesses the individual risk of the specified conditions. After the assessment support, advice and/or treatment will be provided to reduce or manage any identified risk factors.

In some areas this service will be commissioned through community pharmacy, whilst in other areas it is available exclusively through a patient's GP.
