Data Breaches

A personal data breach refers to a breach of security leading to the destruction, loss, alteration, unauthorised disclosure or unauthorised access to personal data. The breach could be intentional or unintentional.

Data breach incidents can include, but are not limited to:

    • Lost or stolen confidential paperwork containing personal data
    • Portable devices containing confidential information lost or stolen
    • Confidential information and personal data sent to the wrong recipient – paper or electronic
    • Confidential information disclosed to an unauthorised person
    • Confidential paperwork placed in general waste rather than confidential waste bin or shredded
    • Amending patient information without consent
    • Unauthorised individuals accessing patient information such as via the PMR
