A customer may request a delivery service either face to face, via telephone, email, freepost reply paid cards or a leaflet at the pharmacy.

It is important that this is fully documented together with any specific requests such as if the delivery is to be to a specified person other than the patient or carer.

A signature must be obtained with the date of the request. You may receive a request from a member of the public whilst you are out, so ensure you are clear on the process they need to follow to request this service from your pharmacy.
