Home medicines reviews for vulnerable older people

This module will look at the benefit of a home medicines review for older people, including medicines optimisation and helping to avoid adverse drug events and non-adherence.

Learning objectives:

• Be able to describe the different factors that impact on medicines optimisation 
• Recognise that being housebound can result in poor engagement with prescribers and dispensers
• Appreciate that polypharmacy increases non-adherence and is the biggest risk for adverse drug events.
